BSA Annual Health and Medical Record form – Parts A and B are required for any person participating in troop activities. Part C (physical exam by physician/provider) is only required for events of 72 hours or more or high adventure bases. The physical is valid until the end of the 12th calendar month following the exam (e.g. if the exam was on May 3, 2023, it’s valid until May 31, 2024).
