Medical & Activity 

• BSA Activity Consent Form

• BSA Annual Medical Form A&B only: Parts A and B are required for any person participating in troop activities.

• BSA Annual Medical Form A,B & C - summer/winter camp: Part C (physical exam by physician/provider) is only required for events of 72 hours or more or high adventure bases. 

Note: The physical is valid until the end of the 12th calendar month following the exam (e.g. if the exam was on May 3, 2026, it’s valid until May 31, 2027).
